Graduate Practical Nurse – Home Care Academy – Private Duty

Vumc is seeking a Graduate Practical Nurse to join our Home Care Academy team in a full-time role serving private duty patients in Nashville, Tennessee. Role Overview

As a Graduate Practical Nurse in our Home Care Academy program, you will deliver compassionate, evidence-based nursing care to patients in their homes across the Nashville area. This role is designed specifically for newly graduated practical nurses, offering structured support and professional development as you transition from education to clinical practice. Working within our private duty home care division, you'll gain real-world experience while building a foundation in patient assessment, care planning, and clinical decision-making under the guidance of experienced preceptors.

Your Responsibilities

• Perform direct patient care activities including vital sign monitoring, wound care, medication administration support, and hygiene assistance
• Document patient assessments and care provided in electronic health records with accuracy and timeliness
• Communicate effectively with patients, families, and the broader healthcare team regarding patient status and care needs
• Follow established protocols and maintain compliance with healthcare regulations and safety standards
• Participate in continuing education and clinical training opportunities to develop nursing competencies
• Support quality improvement initiatives and contribute to positive patient outcomes

Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) in Tennessee or eligibility to obtain licensure
• Completion of an accredited practical nursing program
• Strong communication and interpersonal skills
• Ability to work independently while remaining responsive to supervision and feedback
• Reliable transportation and a valid driver's license
