OhioHealth is hiring a Coder IV to join our Hospital Coding department in the United States. This full-time position supports inpatient coding operations with a focus on accuracy, quality, and compliance in medical record documentation and coding.

The Opportunity

As a Coder IV, you'll play a critical role in translating complex medical documentation into precise, compliant diagnostic and procedural codes. Your work directly impacts hospital operations, quality metrics, and billing accuracy. This position offers the chance to deepen your expertise in hospital coding standards, work alongside experienced coding professionals, and contribute to continuous improvement in clinical documentation practices.

Core Duties

  • Review inpatient medical records and assign appropriate admission, principal, and secondary diagnosis codes according to current coding guidelines and hospital-specific requirements, maintaining quality standards and productivity benchmarks
  • Assign Present on Admission (PO) indicators to all inpatient diagnoses in compliance with official guidelines
  • Accurately assign DRG, MS-DRG, and APR-DRG codes at established quality thresholds
  • Identify and assign CC/MCC indicators and relevant HAC codes when applicable
  • Query physicians when medical documentation is insufficient, ambiguous, or unclear to obtain necessary clarification for coding accuracy
  • Assign transaction codes in the HBOC system for incomplete or conflicting documentation, following departmental escalation procedures
  • Complete UB-04 data abstraction and hospital reporting requirements
  • Verify patient demographics, validate account numbers and charges, and flag missing documentation
  • Identify and escalate problem cases on the DNFB to appropriate staff
  • Collaborate with Clinical Documentation Services to enhance physician documentation and support case mix management

What We're Looking For

  • High school diploma or GED
  • Health Information Management (HIM) degree
  • Certified Coding Specialist (CCS) credential from AHIMA
  • Strong understanding of ICD, CPT, and HCPCS coding standards
  • Ability to consistently meet quality and productivity expectations
  • Attention to detail and commitment to accuracy in a fast-paced environment
  • Ability to work independently and collaborate effectively with clinical and administrative teams
