Troy, OH, Provider Liaison- Medical Assistant Temporary

Theoria Medical is hiring a Medical Assistant for a full-time temporary position in Troy, Ohio. You'll serve as a vital team member in our post-acute care environment, coordinating clinical services, facilitating virtual visits with providers, supporting care transitions, and directly enhancing outcomes for skilled nursing facility residents.

About the Role

In this position, you'll be the bridge connecting residents, physicians, nursing staff, and families within a value-based care model. Your responsibilities center on organizing telemedicine encounters, updating clinical records, preparing residents for provider visits, managing referrals across care settings, and educating patients on health topics. You'll thrive in a dynamic SNF setting where your administrative skills, clinical knowledge, and interpersonal strengths make a measurable difference in resident care quality.

Key Responsibilities

• Schedule and facilitate telemedicine visits, acute appointments, and routine provider follow-ups
• Prepare residents for provider encounters with positioning and clinical documentation support
• Record and maintain medical history information in our electronic health record system
• Coordinate resident care transitions between settings, manage referral processes, and confirm follow-up scheduling
• Process prior authorization requests and support residents with enrollment paperwork
• Educate residents and families on clinical topics including nutrition, fall prevention, medication compliance, and wellness
• Distribute provider-approved educational materials and route clinical concerns to nursing leadership
• Document clinical encounters accurately in the EMR to support regulatory requirements and quality initiatives

Qualifications

• Completion of an accredited Medical Assistant training program
• Certified Medical Assistant (CMA) designation preferred
• Prior experience in a clinical or care-coordination role as a Medical Assistant; health plan or hospice liaison experience is valued
• Demonstrated proficiency charting in electronic health record platforms
• Strong organizational and detail-focused work style
• Solid understanding of geriatric medicine terminology and practices
• Clear verbal and written communication abilities
• Competence with advanced computer functions (data entry, spreadsheet operations, email and web-based systems)
• Self-directed problem-solver who takes initiative and ownership
• Team-oriented, compassionate professional approach
• Ability to stand for extended periods, lift residents independently up to 20 pounds, and assist with heavier transfers within a team setting
• Capability for local travel within a 30-mile radius
• Availability for six-hour shifts typically beginning between 7:00 and 10:00 a.m., with no on-call or overnight responsibilities
