RN (Registered Nurse) Home Health

Phoenix Home Care and Hospice is hiring a full-time Registered Nurse (RN) for our home health team in Clinton, Missouri. In this position, you'll deliver skilled nursing care directly to patients in their homes, combining clinical expertise with compassionate patient support.

What This Role Involves

As an RN in home health care, you'll be the primary healthcare provider for patients recovering from illness or managing chronic conditions within their own homes. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently while coordinating with an interdisciplinary care team. You'll make a meaningful difference in patients' lives by providing evidence-based care and health education that supports their recovery and wellness goals.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans based on physician orders and clinical findings
• Perform skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and other clinical procedures
• Monitor vital signs and patient health status; document observations and report changes to physicians and the care team
• Educate patients and families about medications, treatment plans, disease management, and safety in the home environment
• Maintain accurate, timely medical records and ensure compliance with documentation standards
• Coordinate care with physicians, therapists, social workers, and other healthcare providers
• Respond to patient concerns and provide ongoing support to promote independence and quality of life

Qualifications

• Current, active RN license issued by the State of Missouri
• Minimum of one year of nursing experience in home health or clinical setting
• Proficiency in patient assessment, clinical documentation, and care planning
• Strong communication and interpersonal skills with patients, families, and healthcare teams
• Ability to work independently and manage time effectively across multiple patient visits
• Valid driver's license and reliable transportation; willingness to travel within the service area
• BLS certification (CPR); ACLS certification preferred
