RN (Registered Nurse) Case Manager, Hospice

Join Phoenix Home Care and Hospice as an RN (Registered Nurse) Case Manager in Topeka, Kansas. We're seeking a compassionate, skilled registered nurse to provide coordinated care and support to our hospice patients and their families during their most critical moments.

Position Summary

As a full-time RN Case Manager in our Hospice program, you will be the clinical leader and primary point of contact for your assigned patient caseload. You'll develop individualized care plans, coordinate interdisciplinary team services, assess patient and family needs, and ensure compassionate, dignified end-of-life care aligned with each patient's goals and values. Your clinical judgment, advocacy, and direct patient assessment will be central to delivering excellence in hospice services throughout the Topeka area.

Your Responsibilities

• Perform comprehensive nursing assessments and develop evidence-based hospice care plans for assigned patients
• Coordinate care across the interdisciplinary hospice team, including physicians, social workers, chaplains, and volunteers
• Make regular in-home or facility visits to monitor patient status, manage symptoms, and adjust care as needed
• Educate patients and families about the hospice process, comfort measures, and what to expect
• Communicate clinical updates to the hospice medical director and collaborate on treatment decisions
• Document patient interactions, care plans, and clinical outcomes in compliance with regulatory and organizational standards
• Provide emotional and clinical support to families before, during, and after the patient's death
• Maintain professional licensure and stay current with hospice best practices and continuing education

Who We're Looking For

• Current, unrestricted RN licensure in Kansas
• Minimum 2 years of nursing experience (hospice or palliative care preferred, but not required)
• Strong assessment and critical-thinking skills with ability to identify complex patient needs
• Excellent communication and interpersonal skills; ability to work collaboratively with clinical and support staff
• Genuine empathy and ability to provide compassionate care during sensitive end-of-life situations
• Valid driver's license and reliable transportation for home visits in the Topeka region
• Commitment to ongoing professional development and adherence to nursing standards of practice
