Humber River Health is seeking a dedicated Registered Nurse to join our Medical Day Care program in Toronto, Ontario. This full-time position offers the opportunity to provide essential patient care within a community-focused healthcare setting.
Role Overview
In this Registered Nurse role for Medical Day Care at Humber River Health, you'll deliver direct nursing care to patients participating in our structured day programs. Based in North West Toronto, you'll support patient wellness through clinical assessment, monitoring, and therapeutic care coordination. Your work will be central to ensuring participants receive compassionate, high-quality support within our inclusive day care environment.
Key Responsibilities
- Assess patient health status and develop individualized care plans
- Administer medications and manage clinical protocols in accordance with nursing standards
- Monitor vital signs and track patient health metrics throughout the day
- Maintain accurate and timely clinical documentation and health records
- Collaborate with physicians, allied health professionals, and care staff on patient outcomes
- Provide health education and guidance to patients and families
- Respond promptly to patient needs and report significant clinical changes
- Champion equity and inclusivity in all patient interactions
Skills & Qualifications
- Current Registered Nurse license in Ontario
- Professional nursing registration in good standing
- Demonstrated experience in direct patient care, including day care, community health, or outpatient environments
- Strong clinical assessment and decision-making abilities
- Excellent communication skills and cultural competency
- Experience with electronic health records (EHR) systems
- Commitment to patient-centred, equitable, and inclusive healthcare
