About the Role

• Provide patient-centered care in a community setting, focusing on health education, chronic disease management, and preventive care.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Assess, plan, implement, and evaluate patient care plans.
• Administer medications and treatments as per medical orders.
• Document patient care activities and progress accurately and timely.

Day-to-Day Responsibilities

• Conduct home visits and community health assessments.
• Develop and maintain patient health records.
• Provide health education to individuals and families.
• Support patients and families in managing chronic conditions.
• Refer patients to appropriate health services as needed.

Skills & Qualifications

• Registered Nurse (RN) license in good standing.
• Minimum of 2 years of experience in community health or related field.
• Strong communication and interpersonal skills.
• Proficient in electronic health record systems.
• Ability to work independently and as part of a team.
• Valid provincial health card.
