About the Role
As an ICMS Case Manager, you'll serve clients in our Intensive Case Management Services program within a permanent supportive housing setting. Your work centers on helping people break cycles of homelessness through comprehensive, individualized support. You'll work with clients across every stage—from street outreach through interim and transitional housing to permanent placement—using strategic planning, resourcefulness, and advocacy. This role is based at Villas South Park with a Monday–Friday, 8:00 AM–4:30 PM schedule. Driving to meet clients and access community resources is essential to this position.
What You'll Do
- Conduct intake assessments and develop individualized service plans addressing housing, income, medical, mental health, substance use, vocational, and social goals
- Connect clients with referrals and resources aligned to their service plans
- Administer and interpret standardized assessments such as the VI-SPDAT
- Review client progress at 90 days post-intake and regularly thereafter
- Maintain complete, accurate client files and case notes in compliance with PATH policies and HIPAA
- Collaborate with colleagues, supervisors, and partner agencies to coordinate services

Mandatory Requirements & Conditions of Employment

Compensation: $23.24–$28.28 per hour, paid 24 times annually. Entry-level candidates start at the lower range; those with extensive relevant experience move toward the higher range.
