About the Role
As a Case Manager with PATH's Interim Housing program, you'll advocate for and coordinate services for clients across all phases of their housing journey—from street outreach through permanent supportive housing. Using intensive case management, trauma-informed practice, and motivational interviewing, you'll help each person break the homelessness cycle and achieve lasting stability. The role combines practical service coordination with therapeutic relationship-building and crisis support. Schedule: Monday–Friday, 8:30 a.m. to 5:00 p.m.; driving and reliable transportation required.
What You'll Do
- Conduct intake and needs assessments; partner with clients to develop personalized Individualized Service Plans (ISP) targeting housing, income, health, and social support
- Administer program assessments (VI-SPDAT) and conduct 90-day progress reviews
- Connect clients with healthcare, mental health, benefits, employment, and community resources; ensure engagement and follow-up
- Maintain confidential, complete case files and documentation to HIPAA and PATH standards
- Provide crisis intervention and risk assessment; help clients develop coping skills, safety plans, and self-management capacity
- Build trusting relationships with clients, partners, and stakeholders to expand service reach
- Maintain current knowledge of services and resources throughout Los Angeles County

Compensation and Benefits
$23.24–$28.28 hourly, non-exempt, paid 24 times annually. Benefits include medical, dental, and vision coverage; paid vacation, sick time, and holidays; and a retirement plan.
