About the Role
As part of PATH's Interim Housing team in Los Angeles County, you'll work with clients across multiple stages of housing instability—from street outreach through interim housing, transitional support, and permanent placement. This position centers on meeting people where they are and delivering services tailored to their unique circumstances, strengths, and aspirations. You'll earn $23.24 to $28.28 per hour, with pay distributed over 24 periods per year, working Monday through Friday from 8:30 AM to 5:00 PM.
What You'll Do
- Conduct intake interviews and thorough needs assessments, then work with clients to develop Individualized Service Plans (ISP) that address housing goals, employment, health, substance use support, financial resources, vocational development, and social connections
- Connect clients with community resources, healthcare providers, social services agencies, and support networks aligned with their service plan objectives
- Administer evidence-based assessments such as the VI-SPDAT to prioritize services and guide care planning
- Evaluate client progress and conduct formal 90-day reviews to refine support strategies and celebrate achievements
- Maintain thorough, timely case documentation and client files in compliance with program standards, PATH policies, HIPAA requirements, and applicable legal guidelines
- Develop trusting relationships with clients and community partners that strengthen engagement and increase the likelihood of sustained housing success
- Deliver crisis intervention and risk assessment services that equip clients with coping skills, problem-solving strategies, and tools for independent care management, particularly for individuals experiencing complex or chronic trauma
- Employ motivational interviewing and trauma-informed care approaches in all client interactions and service delivery
- Build and maintain current knowledge of available community resources to provide comprehensive, holistic services

Preferred Experience: Two or more years working in healthcare, case management, homeless services, community outreach, or direct service roles with vulnerable populations.
Education and Qualifications: All education and experience levels are encouraged to apply. You must be authorized to work in the United States; be willing and able to obtain CPR and First Aid certification; and successfully pass a tuberculosis test, background screening, and drug test as a condition of employment. Reliable personal transportation, a valid driver's license, proof of vehicle insurance and ownership, and the ability to qualify for PATH's commercial auto insurance coverage are essential requirements for this position.
