RN – Home Health – Full Time – Evenings – Washington

About the Role

As a Registered Nurse with Mercy's home health service, you'll provide skilled nursing care to patients in their homes during scheduled evening shifts. Working in patients' homes allows you to form meaningful relationships while supporting recovery and wellness management. This part-time role offers schedule flexibility while maintaining the continuity and expertise our patients depend on.

What You'll Do

• Assess patients' physical and emotional health status, including post-hospitalization evaluations and ongoing monitoring
• Develop and implement individualized nursing care plans for patients with diverse medical acuity levels
• Administer IV therapy and maintain access site care with strict attention to infection prevention and safety
• Perform skilled nursing interventions such as wound dressing changes and medication management
• Educate patients and families on self-care, treatment protocols, and health maintenance strategies
• Manage new patient admissions to home health services and complete required clinical documentation
• Conduct patient recertifications to ensure continued appropriateness of home care services
• Coordinate discharge planning and ensure smooth transitions in patient care
• Communicate regularly with physicians, case managers, and interdisciplinary team members

What We're Looking For

• Current Registered Nurse (RN) licensure in Missouri
• Experience in home health, community health, or acute care nursing
• Strong ability to work autonomously while maintaining sound clinical judgment in varied home environments
• Excellent patient and family communication and teaching skills
• Reliable transportation for evening visits throughout the Saint Louis County service area
• Commitment to patient-centered care and professional nursing standards
• Flexibility to work scheduled evening shifts
