About the Role

Adventist Health operates home health and hospice agencies throughout California, Hawaii, Oregon, and Washington, providing personalized in-home care to diverse patient populations. As a Social Services Practitioner with your Master of Social Work (MSW), you'll deliver essential psychosocial support and care coordination directly in patients' homes, helping them navigate complex social and emotional challenges while recovering and managing chronic conditions.

What You'll Do

• Conduct comprehensive psychosocial and functional assessments for home health patients
• Develop and implement individualized care and treatment plans addressing social, emotional, and practical needs
• Provide therapeutic support and advocacy for patients and families throughout their home health journey
• Coordinate services and resources, connecting patients with community supports, financial assistance, and specialized programs
• Collaborate with physicians, nurses, therapists, and other interdisciplinary team members to ensure integrated, person-centered care
• Maintain thorough documentation and communication with all care team members and insurance providers
• Assess and address barriers to recovery, housing stability, medication adherence, and social determinants of health

What We're Looking For

• Master of Social Work (MSW) degree from an accredited program
• Current California Licensed Clinical Social Worker (LCSW) license, or ability to obtain licensure
• Strong clinical assessment, documentation, and communication skills
• Experience in home health, hospice, medical social work, or related healthcare settings preferred
• Compassionate approach to patient care with cultural competency and respect for diverse backgrounds
• Ability to work independently in patients' homes and manage your own caseload
