RN Care Navigator/Case Manager – Remote (NY State RN license required)

About This Opportunity

Our talent team is representing a confidential healthcare organization seeking a Registered Nurse Care Navigator/Case Manager for a fully remote position. This role requires an active New York State RN license and focuses on kidney health case management and care coordination.

The Care Navigator will manage a caseload of mixed acuity members, performing health screenings, medication reconciliation, care planning, and coordinating with internal and external healthcare teams to promote high-quality, cost-effective patient outcomes.

Responsibilities

• Handle inbound and outbound member calls, delivering high-quality service and support
• Educate members on kidney health, related conditions, dialysis options, and transplant pathways
• Engage and enroll members into kidney health programs
• Complete member health and social determinants of health screenings
• Conduct medication reconciliation and maintain member-centric care plans
• Document care activities in the organization's care management system
• Serve as patient advocate, addressing concerns and removing barriers to care
• Coordinate care as liaison between patients, support networks, physicians, and ancillary providers
• Utilize community resources in care planning and coordination
• Follow up with members based on complexity and established protocols
• Notify providers of identified patient needs per policy
• Meet individual metrics and key performance indicators
• Support after-hours coverage and various time zones as needed
• Maintain compliance with HIPAA and all federal, state, and local regulations

What We're Looking For

• Active, unrestricted RN license in New York State (required)
• Bachelor's degree required; alternatively, 6+ years RN experience including 3+ years in case management may be considered
• Three years of case management experience preferred
• CCM certification preferred
• Experience in dialysis or transplant settings preferred
• Experience with Medicare and Medicaid programs preferred
• Proficiency with technical applications including Excel, Outlook, and Word
• Strong verbal and written communication skills
• Ability to thrive in a dynamic environment with changing priorities
• Collaborative mindset with strategic thinking capabilities
• Patient advocacy and self-management support skills
