Psychiatric RN, Care Management

About This Opportunity

We are representing a leading national health insurance organization seeking a Psychiatric RN, Care Management professional to join their telephonic care team. In this remote role, you will assess and support members facing serious mental illness and substance use challenges, working to connect them with the resources and care they need to achieve optimal health outcomes.

This is a fully remote position within the United States, offering the flexibility to work from home while making a meaningful impact on members' lives through compassionate, phone-based care management.

Responsibilities

• Conduct telephonic assessments to identify member needs, barriers to care, risk factors, and access issues for individuals with behavioral health and substance use conditions
• Develop and implement individualized care plans that address both physical and psychosocial health needs
• Educate members and their families on psychiatric treatment options, available resources, and self-management strategies
• Coordinate care across the interdisciplinary team, connecting members to providers, community health agencies, support programs, and local resources
• Monitor patient progress through ongoing assessments and evaluations, adjusting care plans as needed
• Identify and resolve barriers that prevent members from receiving effective care
• Document all interactions and care activities accurately in compliance with HIPAA requirements
• Collaborate with Medicare and Medicaid programs to ensure coordinated benefits and services

What We're Looking For

Required Qualifications:

• Active Registered Nurse (RN) license in good standing
• Minimum 2 years of experience in an integrated care setting managing patients with behavioral health needs and/or substance use disorders
• Experience must include crisis settings such as emergency departments, community mental health centers, behavioral health inpatient units, or assertive community treatment programs
• Ability to obtain and maintain RN licensure in multiple states without restrictions
• Proficiency with Microsoft Office applications including Word, Excel, and PowerPoint
• Dedicated home workspace that ensures privacy and HIPAA compliance
• Reliable internet connection (minimum 25 Mbps download, 10 Mbps upload; wired cable or DSL preferred)

Preferred Qualifications:

• Bachelor's degree in Nursing (BSN)
• Psychiatric nursing certification
• RN license in a compact state
• Experience working with Medicare and/or dual-eligible Medicare/Medicaid populations
• Background in managed care environments
• Spanish language proficiency

Schedule: Eight-hour shifts between 8:00 AM and 6:00 PM in your local time zone; occasional overtime as business needs require. This is a full-time position (40 hours per week) with occasional travel for training or meetings.
