Program Manager (Population Health) – must reside in Michigan

We are representing a confidential healthcare organization seeking a Program Manager (Population Health) to join their Michigan operations. This role focuses on ensuring compliance with state Medicaid contract requirements, managing community-based organization relationships, and developing strategies to address health-related social needs. You must reside in Michigan to be considered for this position.

Responsibilities

• Design and implement population health programs that meet member and community needs while maintaining compliance with state contracts
• Ensure well-documented policies, workflows, program controls, and best practices for assigned programs
• Collaborate with Legal, Compliance, and Information Security to uphold governance standards
• Track performance metrics and ensure value realization from deployed solutions
• Coordinate recurring governance meetings and support decision-making processes
• Support portfolio management and initiative-specific change and project management as directed
• Partner with stakeholders to disseminate and drive adoption of program guardrails, processes, and best practices
• Routinely review and update program collateral to reflect current business needs
• Identify gaps and provide recommendations on program enhancements to leadership
• Create business requirements documents, test plans, requirements traceability matrices, and user training materials
• Generate and distribute standard reports on schedule

What We're Looking For

Required:

• At least 4 years of program and/or project management experience, or equivalent combination of relevant education and experience
• Operational process improvement experience
• Managed care experience, preferably in a shared service, center of excellence, or matrixed environment
• Experience with Microsoft Project and Visio
• Strong presentation and communication skills
• Must reside in Michigan

Preferred:

• Experience with government-sponsored programs (Michigan Medicaid, Medicare, Marketplace)
• Familiarity with value-based care and alternative payment models, community health needs assessment, quality improvement, and social determinants of health
• Experience in quality improvement and provider engagement strategies to improve population health
• Experience evaluating program impact and ROI to inform decision-making
• Experience developing relationships with multidisciplinary teams, key providers, community-based organizations, and healthcare organizations
