Oncology Case Manager – GAU and KZN

Join Discovery Limited as an Oncology Case Manager based in Heidelberg, Lesedi, serving patients across Gauteng and KwaZulu-Natal. As a full-time member of our clinical care team, you'll coordinate comprehensive oncology care pathways and support patients through their cancer treatment journey.

The Opportunity

Discovery Limited is seeking a dedicated Oncology Case Manager to manage clinical care coordination for cancer patients across our Gauteng and KwaZulu-Natal regions. In this role, you'll serve as a vital bridge between patients, healthcare providers, and our clinical team, ensuring seamless care delivery and optimising patient outcomes. Day-to-Day Responsibilities

• Manage oncology patient cases from referral through treatment completion, coordinating all aspects of care
• Liaise with oncologists, treating physicians, and other healthcare providers to facilitate appropriate treatment pathways
• Assess patient needs, develop individualised care plans, and monitor progress throughout treatment
• Manage discovery health benefits, pre-authorisations, and resource allocation for oncology treatments
• Provide patient advocacy and support, addressing concerns and ensuring access to required services
• Maintain comprehensive case records and clinical documentation in line with regulatory requirements
• Identify barriers to care and implement solutions to optimise patient engagement and compliance

What We're Looking For

• Nursing qualifications with previous case management experience, preferably in oncology
• Strong understanding of oncology treatments, cancer pathways, and patient management principles
• Excellent communication skills with patients, families, and medical professionals
• Ability to manage complex clinical cases independently with sound clinical judgment
• Proficiency with health information systems and electronic case management tools
• Knowledge of South African healthcare systems, medical aid schemes, and regulatory frameworks
• Compassionate approach with emotional resilience for working with critically ill patients
