The State of South Carolina Department of Social Services seeks a dedicated CFTM Facilitator for its office in Market, Richland County. This full-time role offers a meaningful opportunity to serve children, families, and vulnerable adults through collaborative, community-centered support and coordination.

Position Summary

As a CFTM Facilitator, you are the bridge between families and community resources. You'll lead collaborative team meetings, guide service planning with family input, and help coordinate the supports that enable families to achieve stability and well-being. In this role, you directly advance South Carolina's mission to protect and strengthen vulnerable populations through person-centered practice.

What You'll Do

• Facilitate Child and Family Team meetings where families, providers, and agency staff collaborate on goals and plans
• Engage families as equal partners in identifying strengths, needs, and solutions
• Coordinate communication and service delivery across multiple agencies and community providers
• Support the development, implementation, and adjustment of family service plans
• Connect families with community resources that address their identified needs
• Maintain thorough, accurate documentation of meetings and outcomes
• Build and maintain respectful, trust-based relationships with diverse families and partners
