About the Role
You'll serve as a critical team coordinator within the Department of Social Services' family preservation initiative. Your focus will be ensuring that families receive coordinated, compassionate support to remain safely together while protecting the well-being of children and vulnerable adults in the community.
What You'll Do
- Coordinate and support family preservation services delivery within your assigned team
- Partner with caseworkers, supervisors, and community organizations to develop and implement family-centered service plans
- Monitor progress and track outcomes for families engaged in preservation services
- Facilitate communication and collaboration between internal teams and external service providers
- Maintain accurate documentation and contribute to program data collection and reporting
- Support crisis intervention and family stabilization efforts
- Assist with training initiatives, quality assurance, and continuous program improvement
- Help connect families with resources and services to address underlying needs
