PATH (People Assisting the Homeless) is hiring a full-time Case Manager for its Housing Navigation program in Los Angeles County. About the Role

Based in the Los Angeles area, this full-time position operates Monday through Friday, 8:00 a.m. to 4:30 p.m. You'll work directly with program participants, starting with thorough intake assessments and moving forward to create tailored action plans that address each person's unique housing needs, employment barriers, health concerns, and personal goals. Your mission is to guide clients toward independent living and sustained housing security.

What You'll Do

  • Conduct in-depth intake conversations and needs assessments to understand each participant's situation, challenges, and aspirations
  • Create personalized roadmaps addressing housing acquisition, income and employment, medical and mental health needs, substance use recovery, financial stability, and community connections
  • Administer standardized screening and assessment instruments to evaluate progress and identify ongoing barriers
  • Identify appropriate community resources and coordinate referrals to partner agencies, housing providers, employers, health services, and support organizations
  • Conduct structured progress evaluations at key intervals, including comprehensive 90-day reviews after program enrollment
  • Maintain detailed, well-organized participant files that comply with program requirements and organizational standards
  • Protect confidential participant information and ensure full compliance with HIPAA and applicable privacy laws

Required Qualifications

  • Authorized to work in the United States
  • Current CPR and First Aid certification, or willingness to earn certification as a condition of employment
  • Valid driver's license and access to reliable personal transportation with documented proof of insurance and vehicle ownership
  • Ability to pass background screening, drug screening, and tuberculosis testing prior to hire

Compensation and Benefits

This position offers a salary range of $23.24 to $28.28 per hour, scaled according to your relevant experience. Benefits include medical, dental, and vision coverage, paid vacation and sick leave, paid holidays throughout the year, and access to a retirement plan.
