What This Role Involves

As an AFL Provider with Easterseals PORT, you'll become part of a person's daily life—offering a safe, welcoming home where they can thrive. You'll provide 24-hour care, support, and habilitation services tailored to each individual's unique needs and goals. This role combines hands-on caregiving, life skill development, community integration, and advocacy within a residential family setting.

Core Duties

• Provide safe, supportive 24-hour residential care in your home
• Develop and implement individualized habilitation plans aligned with each person's goals
• Assist with daily living activities (meals, personal hygiene, household management)
• Support community involvement, social connections, and recreational activities
• Monitor health, wellbeing, and behavioral support needs
• Maintain detailed records and communicate progress to the care team
• Collaborate with Easterseals PORT staff, therapists, and family members
• Advocate for the individual's rights, preferences, and dignity

Skills & Qualifications

• Compassion, patience, and genuine commitment to supporting individuals with developmental disabilities
• Strong communication skills and ability to work as part of an interdisciplinary team
• Ability to manage challenging situations with calm, problem-solving focus
• Flexibility and willingness to adapt to individual needs and schedules
• High school diploma or GED (or equivalent experience demonstrating capability)
• Valid driver's license and reliable transportation
• Willingness to undergo background screening and training
• Physical ability to assist with activities of daily living as needed
