Medical Social Worker – Home Health Visits

BAYADA Home Health Care is hiring a full-time Medical Social Worker to deliver home-based clinical and social support services in the Baltimore area, including Rosedale, MD 21237. You'll serve adult and geriatric patients on a per-visit basis throughout East Baltimore County and Baltimore City.

About the Role

As a Medical Social Worker, you'll be a vital member of our multidisciplinary team that enables clients to receive quality medical care at home. Your clinical expertise will help patients and families navigate the social and emotional dimensions of health, connect with essential community resources, and build sustainable recovery plans centered on independence and wellness.

What You'll Do

• Conduct clinical home visits with assigned patients across East Baltimore County and Baltimore City, including Dundalk, Sparrows Point, Essex, and Middle River.
• Perform comprehensive social assessments to understand factors influencing each patient's health and well-being.
• Develop intervention plans grounded in your clinical evaluation findings.
• Identify and refer patients to appropriate community services and support resources.
• Advocate for patients and families, helping them navigate local health and social systems.
• Collaborate with nursing and rehabilitation staff to integrate social work into the patient's care plan.
• Build relationships with community agencies to ensure resource availability for your patients.
• Educate patients and families about community services that support health, safety, and independence.
• Document clinical observations, interventions, and assessments using mobile technology.

Skills & Qualifications

• Valid LMSW or LISW licensure from the Maryland Board of Social Work Examiners.
• Master's degree in Social Work from an accredited program.
• Minimum 2 years of professional community-based social work experience.
• Independent work habits and strong time management skills.
• Excellent interpersonal and written communication abilities.
• Comfort with technology; electronic medical records (EMR) experience preferred.
• Reliable transportation and ability to travel to patient locations.
• Prior home care experience is valued.
