Outpatient Home Health Physical Therapist

BAYADA Home Health Care is hiring a full-time Outpatient Home Health Physical Therapist to serve senior residents in Newton and Weston, Massachusetts. This position offers the opportunity to make a direct impact on older adults' quality of life through evidence-based therapy delivered within their communities.

What This Role Involves

As an Outpatient Home Health Physical Therapist with BAYADA Home Health Care, you'll provide skilled therapeutic services to geriatric residents of assisted living and independent living communities across Newton, MA and the surrounding region. Working within a collaborative interdisciplinary team, you'll help older adults maintain their independence and age safely in place through individualized physical therapy interventions and education.

Core Duties

• Conduct outpatient therapy visits at designated senior living communities in Newton and Weston
• Perform comprehensive clinical assessments including range of motion, strength and endurance testing, gait and balance evaluation, transfer capability analysis, and functional safety screening
• Create individualized treatment plans incorporating therapeutic exercise, gait training, balance work, and movement education tailored to each resident's goals
• Regularly update and adjust care plans based on client progress; participate in team meetings with community staff and multidisciplinary healthcare professionals
• Provide instruction and guidance to residents, family members, and community care staff on safe movement, independence strategies, and therapy compliance
• Document all clinical findings, treatments rendered, and outcomes using modern electronic charting systems

Skills & Qualifications

• Valid, current Massachusetts Physical Therapist license
