Home Health Physical Therapist – $68 per visit

BAYADA Home Health Care is looking for a dedicated Physical Therapist to join our clinical team in Elkins Park, Montgomery County. This is a full-time position providing skilled physical therapy services in clients' homes throughout the region.

The Opportunity

As a Home Health Physical Therapist with BAYADA, you'll deliver one-on-one patient care in a flexible, independent setting. Working with clients in their own homes allows you to build meaningful relationships while helping them achieve their rehabilitation goals. Compensation is structured at $68 per visit, with the flexibility of a full-time position. You'll use clinical judgment to design and implement individualized treatment plans, measure progress, and adapt care based on each patient's needs and home environment.

Responsibilities

• Conduct comprehensive physical therapy assessments and develop personalized treatment plans for home-based patients
• Execute evidence-based therapeutic interventions and skilled exercises during in-home visits
• Educate patients and caregivers on therapeutic techniques, safety strategies, and home exercise programs
• Monitor patient progress, document clinical outcomes, and adjust treatment as appropriate
• Collaborate with physicians, nurses, and other care team members to coordinate comprehensive patient care
• Maintain accurate, timely clinical documentation and comply with regulatory standards

Requirements

• Current Pennsylvania Physical Therapy license (PT)
• Bachelor's degree in Physical Therapy or equivalent; Doctor of Physical Therapy (DPT) preferred
• Experience in home health, skilled nursing, or acute care settings preferred
• Valid driver's license and reliable transportation
• Strong communication and patient education skills
• Ability to work independently and manage a patient caseload
