Bilingual Field Care Coordinator / Field Case Management

About This Opportunity

We are representing a confidential Medicaid and Medicare healthcare organization in the Bay Area that is expanding its field-based care coordination services. Our client is seeking a Bilingual Field Care Coordinator to bring whole-person care directly to patients throughout Alameda County.

This is a high-impact, community-based role where you will work independently to support patients with complex medical, behavioral, and social needs in their homes and neighborhoods. You'll complete approximately 30 in-person patient visits per week (up to 120 per month), managing a busy schedule while traveling throughout Alameda County.

Schedule & Travel: Monday–Friday, 9:00 AM–5:30 PM. You will be in the community 5 days per week, traveling approximately 30–40 miles per day. Weekly travel to and around Oakland is required. Gas and mileage reimbursement provided.

Responsibilities

• Manage a high-volume, field-based caseload and complete approximately 30 in-person patient visits per week throughout Alameda County
• Independently manage daily schedules while meeting patients where they are—at home, in the community, or at partner organizations
• Serve as the primary point of contact for members with complex medical, behavioral, and social needs
• Conduct in-person visits for new patient onboarding, care plan follow-up, health assessments (including blood pressure checks), and delivery of care packages or resources
• Coordinate services, appointments, and referrals across primary care, behavioral health, and social support systems
• Collaborate with internal care team members including nurses and behavioral health care managers to ensure continuity of care
• Identify and address barriers to care such as transportation, medication access, or health literacy challenges
• Develop and maintain individualized care plans with ongoing follow-up on all interventions and goals
• Support recurring community-based events and provide care coordination for patients with complex needs
• Maintain accurate and timely documentation of all member interactions and care coordination activities
• Participate in collaborative care meetings to review patient panels and align on care strategies

What We're Looking For

• Bilingual – English/Spanish required
• 1+ years of field-based (in-person) case management or care coordination experience
• Physical location in or near Alameda County with familiarity of local community resources
• Reliable transportation for regular field visits throughout the county
• Valid driver's license and current auto liability insurance
• Strong cultural fluency and ability to build trust with diverse populations
• High degree of empathy and patience when faced with adversity
• Excellent organizational skills and attention to detail with ability to manage a fast-paced, high-volume workload
• Strong technical skills and comfort learning new systems (Google Suite, Slack, CRM databases, Excel, Word, email, video conferencing)
• Reliable, HIPAA-compliant workspace with stable internet connection for documentation and team communication
• Ability to work collaboratively in a multidisciplinary care team

Pay

Pay via Get A Job.ai: $32/hr
