Case Manager, Registered Nurse – Remote

About This Opportunity

We are representing a confidential healthcare organization seeking a Remote Case Manager, Registered Nurse to join their team. This is a full-time, work-from-home position based in the United States (Ohio residency preferred). Our client is looking for an experienced RN who can coordinate comprehensive care, advocate for patients, and ensure seamless care transitions across healthcare settings.

Responsibilities

• Administer care coordination plans to assess patient needs and facilitate smooth transitions between care settings
• Analyze complex patient data including medical history, diagnostic results, and treatment plans to understand current health status
• Organize and maintain patient files in compliance with HIPAA guidelines for easy retrieval and continuity of care
• Communicate through secure internal platforms to exchange messages, conduct video conferences, and collaborate on patient care plans
• Conduct routine utilization reviews to ensure patients receive appropriate, cost-effective care
• Manage case management systems to track patient progress, organize cases, and monitor specific conditions
• Coordinate discharge planning processes to ensure safe and timely transitions from hospital to home
• Develop and optimize resource management to connect patients with community healthcare resources
• Generate reports and analyze data on key performance health indicators

What We're Looking For

• Active and unrestricted Registered Nurse Licensure in your state of residence (required)
• Associate's Degree in Nursing (required); Bachelor's Degree preferred
• 5+ years of relevant nursing and case management experience
• Strong knowledge of medical terminology and case management practices
• Excellent problem-solving and decision-making skills
• Digital literacy and comfort with healthcare technology platforms
• Ability to handle sensitive patient information ethically and in compliance with HIPAA
• Strong communication skills and ability to work tactfully with diverse patient populations
• Experience evaluating costs and benefits to determine most appropriate care options
• Dedicated home workspace that meets work-from-home requirements: hardwired internet connection (minimum 25mbps download/3mbps upload), quiet and secure workspace, HIPAA-compliant environment
