The Opportunity
This leadership role offers the chance to guide and develop a team of community health workers who deliver care where people live—in homes, neighborhoods, and community spaces. You'll manage frontline staff providing patient-centered support, coordinate programs with partner organizations, and ensure every member receives the culturally aligned, accessible care they deserve. This position is ideal for someone passionate about health equity who wants to make a direct impact on underserved communities.
Key Responsibilities
- Supervise and coach a team of Promotoras de Salud (health promoters), conducting regular one-on-one check-ins and performance evaluations
- Monitor team attendance, performance against program goals, and provide feedback to improve clinical and operational outcomes
- Develop and maintain quality assurance processes, including documentation standards and performance metrics
- Facilitate team case conferences to discuss client needs, share best practices, and provide peer support
- Support staff in patient assessments, care coordination, and appropriate clinical or social service referrals
- Serve as an on-call resource to troubleshoot issues and escalate complex situations to leadership
- Contribute to program improvements by gathering member feedback and identifying operational enhancements
- Collaborate with community partners, healthcare providers, and health plans on care model updates
Qualifications
- Bachelor's degree or equivalent professional experience in social services, public health, or related field
- 3+ years working in a healthcare setting (ambulatory care, community health, public health, or similar)
- 2+ years of direct community health worker or health promoter experience
- 2+ years of supervisory or team lead experience with frontline staff
- Hands-on experience with patient care coordination, advocacy, and healthcare navigation
- Understanding of Medicaid and Medicare coverage and eligibility
- Fluency in English and Spanish (verbal and written)
- Strong written and verbal communication skills with ability to listen deeply and communicate clearly across roles
- Demonstrated ability to lead with calm, consistency, and sound judgment in complex or high-pressure situations
- Proficiency with computer systems and standard office technology
- Ability to maintain strict confidentiality and follow HIPAA protocols
