Join The Children's Village as a Case Planner in our Supervised Independent Living Program, based in Manhattanville, Manhattan. In this full-time position, you'll provide comprehensive case management and supportive services to help young adults build the skills and stability they need to thrive independently.

About the Role

As a Case Planner in our SILP, you'll serve as a trusted guide for youth in residential care, managing all aspects of their case from intake through transition to independent living. You'll assess individual needs, develop tailored service plans, connect residents to community resources, and provide hands-on counseling and support. Working as a core member of the SILP team, you'll collaborate across departments to ensure each young person achieves measurable progress toward their independence and life goals.

What You'll Do

  • Conduct comprehensive assessments of youth in the program and develop individualized case plans aligned with their strengths and goals
  • Coordinate services including counseling, employment support, education, healthcare, and housing resources
  • Provide crisis support, advocacy, and coaching to help residents navigate challenges and build resilience
  • Maintain accurate and timely documentation in compliance with program standards and regulatory requirements
  • Facilitate connections between residents and community services, employers, educational institutions, and social networks
  • Prepare youth for successful transitions to independent living through skills training and ongoing follow-up support
  • Participate in team meetings, case consultations, and program development activities

What We're Looking For

  • Bachelor's degree in social work, human services, psychology, or a related field
  • Prior experience in case management, youth services, or residential program coordination
  • Strong interpersonal and communication skills with the ability to build trust with young people and families
  • Working knowledge of New York State social services systems, youth welfare services, and available community resources
  • Ability to work with diverse, often vulnerable populations with cultural sensitivity and respect
  • Excellent organizational and documentation skills
  • Valid driver's license and reliable transportation
