About the Role
The SSVF Case Manager position sits at the heart of our retention efforts. You'll work within our established support systems to help clients maintain housing stability and overcome barriers to long-term success. This is hands-on work that demands commitment, resourcefulness, and genuine empathy for people navigating complex life circumstances.
What You'll Do
- Conduct comprehensive intake assessments and collaborate with each client to create personalized service plans aligned with their housing, income, medical, mental health, and personal goals
- Administer program-specific screening tools (such as VI-SPDAT) to evaluate client needs and guide service delivery
- Connect clients with community resources, benefits programs, and support services that advance their goals toward sustained housing stability
- Conduct structured 90-day reviews to track progress and adjust service plans as needed
- Maintain detailed, accurate case documentation in compliance with HIPAA and program standards, ensuring all client files remain current and complete
- Use a "whatever it takes" problem-solving mindset to address barriers and help clients move forward
What We're Looking For
- Demonstrated ability to listen actively, communicate clearly (verbally and in writing), and build trust across diverse populations
- Strong analytical skills with careful attention to detail in documentation and case management tasks
- Proactive problem-solver who can identify creative solutions and take initiative without constant direction
- Commitment to ethical practice, professional boundaries, and confidentiality in all client interactions
- Ability to work collaboratively with team members, partner agencies, and stakeholders toward shared goals
- Valid driver's license and reliable personal transportation (driving is essential to this position)
- Preferred: 2+ years of experience in case management, healthcare, homeless services, or similar fields working with vulnerable populations

What Comes With the Role
This full-time, non-exempt position offers an hourly rate of $23.24 to $28.28, paid over 24 periods annually. Requirements to Move Forward
- Current employment eligibility to work in the United States
- Proof of insurance and vehicle ownership for personal vehicles used during work duties

- CPR and First Aid certification (or willingness to obtain)
- Successful completion of tuberculosis screening, background check, and drug screening as a condition of employment
