What You'll Do Conduct comprehensive intake assessments and collaborate with each client to create personalized service plans aligned with their housing, income, medical, mental health, and personal goals Administer program-specific screening tools (such as VI-SPDAT) to evaluate client needs and guide service delivery Connect clients with community resources, benefits programs, and support services that advance their goals toward sustained housing stability Conduct structured 90-day reviews to track progress and adjust service plans as needed Maintain detailed, accurate case documentation in compliance with HIPAA and program standards, ensuring all client files remain current and complete Use a "whatever it takes" problem-solving mindset to address barriers and help clients move forward What We're Looking For Demonstrated ability to listen actively, communicate clearly (verbally and in writing), and build trust across diverse populations Strong analytical skills with careful attention to detail in documentation and case management tasks Proactive problem-solver who can identify creative solutions and take initiative without constant direction Commitment to ethical practice, professional boundaries, and confidentiality in all client interactions Ability to work collaboratively with team members, partner agencies, and stakeholders toward shared goals Valid driver's license and reliable personal transportation (driving is essential to this position) Preferred: 2+ years of experience in case management, healthcare, homeless services, or similar fields working with vulnerable populations All education and experience levels are encouraged to apply About PATH (People Assisting the Homeless) Since 1984, PATH has pioneered innovative, evidence-based services to support people experiencing homelessness across California.
