Shallow Subsidy Case Manager – (JR 5546)

PATH (People Assisting the Homeless) is hiring a full-time Shallow Subsidy Case Manager in Los Angeles, CA to join our mission of connecting people experiencing homelessness with stable housing and supportive services that transform lives.

About the Role

In this position, you'll work directly with participants in our Shallow Subsidy program, using a "whatever it takes" approach to help individuals break the cycle of homelessness and achieve lasting housing stability. You'll be the cornerstone of our client-centered case management model, conducting thorough assessments, developing personalized service plans, and connecting people with the resources they need to succeed. This is Monday through Friday, 8am to 4:30pm work based at our Los Angeles location, and you'll need reliable transportation, as driving to meet clients and coordinate services is an essential part of the job.

What You'll Do

• Complete intake sessions and individualized needs assessments for program participants, identifying barriers related to housing, income, health, mental health, substance use, finances, employment, and social support
• Develop and maintain Individualized Service Plans (ISP) that reflect each participant's unique goals and strengths
• Administer standardized assessments such as the VI-SPDAT to gather comprehensive client data
• Connect participants with community resources, services, and referrals aligned to their ISP goals
• Conduct 90-day progress reviews to evaluate outcomes and adjust support strategies
• Maintain accurate, timely, and confidential case documentation according to PATH policies and HIPAA requirements

• Collaborative team player who builds positive relationships and values diverse perspectives
• Proactive problem-solver with creativity and willingness to think beyond conventional solutions
• Strong ethical compass and professional boundaries in all client interactions
• Preferred: 2+ years of experience in case management, healthcare, homeless services, or working with vulnerable populations
• Required: Valid driver's license, proof of insurance and vehicle ownership, ability to obtain CPR/First Aid certification, and willingness to pass a background check, TB test, and drug screening
