About the Role

As an Outreach Case Manager with our SSVF team, you'll work with participants to break cycles of homelessness through individualized, comprehensive support. Your role combines assessment, care planning, resource navigation, and documentation to help clients reach their housing and personal goals.

What You'll Do

  • Conduct client intake interviews and complete individualized needs assessments, identifying barriers related to housing, income, health, substance use, employment, and social support
  • Develop and collaborate on individualized service plans aligned with each participant's specific goals and circumstances
  • Administer and score standardized assessments such as the VI-SPDAT to guide service planning
  • Connect participants with community resources, benefits programs, and supportive services that address their identified needs
  • Perform structured 90-day progress reviews to evaluate goal achievement and adjust service plans as needed
  • Maintain accurate, timely case documentation in compliance with program standards, PATH policies, and HIPAA regulations
  • Ensure participant confidentiality and secure all sensitive information according to legal and regulatory requirements

What We're Looking For

You'll bring strong interpersonal and professional competencies including:

  • Excellent communication skills (verbal and written) to engage diverse clients, colleagues, and community partners effectively
  • Analytical abilities with strong attention to detail and capacity to gather, interpret, and apply data to inform decisions
  • Collaborative mindset with ability to build positive working relationships and contribute to team goals
  • Proactive approach with creative problem-solving skills and willingness to take initiative
  • Sound judgment and ethical standards with commitment to professional boundaries and integrity
  • Preferred: Two or more years of experience in case management, healthcare, homeless services, or work with vulnerable populations

Requirements

  • Valid U.S. work authorization
  • Current or willingness to obtain CPR/First Aid certification
  • Valid driver's license and reliable personal transportation for work duties
  • Proof of vehicle insurance and ownership
  • Ability to meet PATH's vehicle insurance requirements
  • Must pass tuberculosis screening, background check, and drug screening prior to hire
