Join Hartford HealthCare at Home as a Full-Time Wound Care Licensed Practical Nurse (LPN) in East Hartford, Hartford County, Connecticut. You'll deliver compassionate, expert wound management care to patients in their homes as part of Connecticut's largest home healthcare network.

About the Role

As a Wound Care LPN at Hartford HealthCare at Home, you'll play a critical role in patient recovery and comfort. Working from our East Hartford location (60 Hartland St), you'll travel to patient homes throughout the service area to provide direct, hands-on wound care and nursing support. This full-time position offers the opportunity to make a meaningful impact on patient outcomes while being part of a healthcare organization where every moment matters.

What You'll Do

  • Assess and monitor patient wounds, documenting healing progress and changes
  • Perform skilled wound dressings and care according to clinical protocols
  • Collaborate with registered nurses, physicians, and care teams to optimize treatment plans

  • Maintain accurate, timely clinical documentation in patient records

What We're Looking For

  • Current CPR certification (BLS or equivalent)
