Social Worker (SW) – SNF

Aequor Healthcare Services, Llc. is seeking a Social Worker (SW) to join our skilled nursing facility in Westwood, Hamilton County. This is a part-time position offering days shift work in a collaborative healthcare setting.

What This Role Involves

As a Social Worker in a skilled nursing facility (SNF) environment, you'll deliver comprehensive psychosocial support and care coordination to residents. You'll assess individual needs, advocate for resident welfare, and connect people with community services and resources. Your work directly improves resident quality of life and supports successful care transitions.

Responsibilities

• Conduct psychosocial assessments for new and current residents to identify support needs
• Develop, implement, and maintain individualized social service care plans aligned with facility standards
• Coordinate discharge planning and connect residents with community resources and referral services
• Facilitate family conferences and serve as liaison between residents, families, and the care team
• Address behavioral, emotional, and adjustment concerns with clinical expertise and compassion
• Document assessments and interventions accurately in resident medical records
• Participate in interdisciplinary care conferences and treatment planning sessions
• Ensure adherence to state and federal SNF regulations and social work standards of practice

What We're Looking For

• Master's degree in Social Work (MSW) or equivalent qualification
• Current state licensure as an LCSW, LMSW, or LSW per regulatory requirements
• Prior experience in skilled nursing facilities or long-term care settings
• Excellent communication and relationship-building skills with residents, families, and interdisciplinary teams
• Ability to work independently while collaborating across departments
• Knowledge of community resources, discharge planning, and social service systems
• Commitment to person-centered care and resident advocacy
