Remote Patient Care Coordinator (US Hours)

Our talent team at Get A Job.ai is representing a confidential telehealth organization seeking a Remote Patient Care Coordinator to work US business hours from the Philippines. This is a full-time independent contractor position supporting patients through their healthcare journeys with empathy, precision, and advocacy.

In this role, you'll serve as the primary point of contact for patients accessing telehealth services, clinical research studies, and specialty care for complex chronic conditions. You'll guide families through healthcare systems, coordinate care across clinical teams, and provide compassionate support during vulnerable moments. This position requires exceptional communication skills, healthcare background, and the ability to navigate complex patient needs with professionalism and genuine care.

Responsibilities

Patient Support & Communication (50%)

• Act as first line of contact for patient inquiries across EHR, secure chat, phone, and messaging platforms
• Respond to inbound patient communications promptly and empathetically
• Educate patients about telehealth services, clinical trials, and specialty care options
• Handle scheduling, appointment coordination, prescription management, and follow-ups
• Address patient concerns with personalized, compassionate solutions and active listening
• Ensure timely resolution of patient inquiries through clinical team collaboration

Care Coordination & Patient Navigation (40%)

• Conduct initial outreach to patients interested in clinical trials or specialty care services
• Administer pre-screener questionnaires and qualification assessments
• Guide patients through onboarding, explaining services, requirements, and addressing questions
• Coordinate with clinical teams and external providers for diagnostic testing, imaging, and referrals
• Manage prior authorizations, insurance coordination, and healthcare system navigation
• Track patient progress ensuring continuity of care throughout their journey
• Send appointment reminders, follow-ups, and medication adherence communications
• Serve as patient advocate during challenging moments and complex healthcare situations

Documentation & Data Management (5%)

• Accurately log patient interactions and progress in CRM and EHR systems
• Document patient encounters and physician notes in EMR as Medical Scribe
• Maintain HIPAA-compliant documentation and confidential patient records
• Track patient data systematically for reporting and care coordination purposes

Cross-Functional Collaboration & Process Improvement (5%)

• Collaborate with operations, clinical, and product teams across departments
• Provide insights and feedback to improve processes and enhance patient experience
• Participate in continuous process improvement efforts and team meetings
• Create automations to reduce manual tasks and improve operational efficiency

What We're Looking For

Required Qualifications:

• 1-2+ years in patient support, healthcare, clinical research, telemedicine, care coordination, case management, or phone-based customer service
• Prior experience in US healthcare settings with understanding of healthcare systems, insurance, referrals, and care processes
• Excellent written and verbal English communication skills with compassionate, empathetic approach
• Familiarity with medical terminology and clinical documentation
• Knowledge of HIPAA regulations, data protection standards, and confidential information handling
• High level of tech proficiency navigating CRM, EHR/EMR systems, and data management platforms
• Exceptional organizational skills and attention to detail managing multiple patient interactions
• Strong creative problem-solving and troubleshooting abilities
• Ability to handle sensitive information and challenging situations with professionalism
• Proven ability to manage complex projects and multiple priorities in fast-paced environments
• Must work US business hours from the Philippines

Preferred Qualifications:

• Good Clinical Practice (GCP) certificate
• 1+ year experience as Medical Scribe in clinical or telehealth settings
• Experience with Elation Health EHR or similar digital health platforms
• Understanding of clinical research standards, trial protocols, and patient recruitment
• Knowledge of rare and complex chronic conditions (EDS, POTS, MCAS, etc.)
• Experience in healthcare startups or digital health companies
• Spanish language proficiency for bilingual patient communication

Technical Skills Required:

• EHR/EMR Systems: Elation Health or equivalent electronic medical record platforms
• CRM Platforms: Salesforce, HubSpot, or healthcare-specific CRM systems
• Google Workspace (Gmail, Sheets, Drive, Docs)
• Video conferencing platforms (Zoom or equivalent)
• Secure patient messaging and communication platforms
