This full-time, 8-hour daytime position offers the opportunity to provide expert clinical pharmacy services to patients managing complex autoimmune and digestive disorders.

The Opportunity

The Pharmacotherapy Clinic at JMH seeks a dedicated clinical pharmacist to optimize medication therapy for patients with rheumatologic and gastrointestinal diseases. Working standard day hours in our Walnut Creek location, you'll collaborate with physicians, nurse practitioners, and interdisciplinary teams to deliver evidence-based pharmacotherapy recommendations. Core Duties

• Conduct comprehensive medication therapy reviews for rheumatology and GI patients
• Provide clinical recommendations to optimize therapeutic outcomes and minimize adverse effects
• Monitor patient response to biologic therapies, disease-modifying antirheumatic drugs (DMARDs), and GI-specific medications
• Collaborate with the clinical team on treatment plans and dosage adjustments
• Document clinical assessments and interventions in patient records
• Support patient education regarding medication use, side effects, and adherence
• Participate in clinic operations and contribute to quality improvement initiatives

What We're Looking For

• Doctor of Pharmacy (PharmD) degree from an accredited institution
• Active pharmacist license in California
• Demonstrated clinical experience or specialized training in rheumatology and/or GI pharmacotherapy
• Strong clinical knowledge of immunosuppressive agents and GI therapeutics
• Excellent communication and interpersonal skills
• Ability to work effectively within a collaborative healthcare team
• Commitment to evidence-based practice and continuing education
