Community Care Of West Virginia is seeking a compassionate, mission-driven Licensed Graduate Social Worker (LGSW) to join our care team in New Martinsville, Wetzel County. In this full-time role, you'll champion the emotional and social wellbeing of adolescent and adult patients across our community.

About the Role

As a Licensed Graduate Social Worker (LGSW) with Community Care Of West Virginia, you will support the medically related emotional and social needs of patients on a personalized, individual basis. This on-site position in New Martinsville carries a tentative start date of September 2026, and centers on delivering care that aligns with established organizational policies and current federal, state, and local standards, guidelines, and regulations.

Key Responsibilities

• Assess and respond to the emotional and social needs of both adolescent and adult patients tied to their medical care.
• Develop and maintain individualized care approaches tailored to each patient's circumstances.
• Coordinate social work services in accordance with company policies, procedures, and applicable regulatory requirements.
• Collaborate with the broader care team to promote patient wellbeing and continuity of support.
• Maintain accurate documentation consistent with federal, state, and local compliance standards.

Qualifications

• Active Licensed Graduate Social Worker (LGSW) credential.
• Working knowledge of social work practice with adolescent and adult populations.
• Familiarity with relevant federal, state, and local healthcare and social services regulations.
• Strong interpersonal, assessment, and communication abilities.
• Availability to work full-time, on-site in New Martinsville, Wetzel County.
