About the Role
As a Housing Navigator, you'll be the bridge between people experiencing housing instability and the resources they need to rebuild their lives. Working Monday through Friday from 8:00 a.m. to 4:30 p.m., you'll conduct comprehensive intake assessments, develop individualized service plans, and maintain ongoing support to help participants achieve their housing goals. This role requires reliable transportation and a valid driver's license, as you'll meet with clients both on-site and throughout the Long Beach and Los Angeles County service area.
What You'll Do
- Perform intake assessments and collaborate with participants to create personalized service plans addressing housing, income, healthcare, mental health, substance use, financial stability, and social support needs
- Connect participants with referrals, benefits resources, and community support services aligned to their individual goals
- Administer standardized assessment tools to identify vulnerability and service needs
- Conduct 90-day progress reviews and adjust service plans based on outcomes and participant priorities
- Maintain complete, accurate case files with timely documentation while ensuring HIPAA compliance and confidentiality
- Provide crisis intervention and risk assessment services, supporting clients in developing problem-solving skills and emotional resilience
- Apply motivational interviewing and trauma-informed practices in all client interactions
- Build and maintain strong working relationships with colleagues, community partners, and the participants you serve

Requirements
- Authorization to work in the United States
- Valid driver's license and reliable transportation
- Proof of vehicle insurance and ownership
- CPR/First Aid certification (or willingness to obtain it)
- Pass a background check, tuberculosis screening, and drug test
- Ability to qualify for PATH's insurance coverage
