About the Role
This is a full-time, day-shift opportunity for a Certified Nursing Assistant who wants to deliver compassionate, hands-on care in a homecare setting. As a Hospice Homecare CNA with Agrace, you will travel to patients in and around the Waukesha area, providing the personal attention and dignity that end-of-life care demands. The schedule is Monday through Friday, 8:00 AM to 4:30 PM (1.0 FTE / 40 hours per week), with weekend coverage every twelfth week and three holiday rotations per year. Starting pay is $18.50 based on experience, plus a $3,000 sign-on bonus.
Key Responsibilities
- Provide skilled personal care to hospice patients in their homes, including bathing, grooming, mobility assistance, and daily living support.
- Travel reliably to scheduled patient visits across the Waukesha service area.
- Observe and report changes in each patient's condition to the care team.
- Offer comfort, companionship, and respect to patients and their families during sensitive moments.
- Document care accurately and follow each patient's individualized plan of care.
Qualifications
- Active Certified Nursing Assistant (CNA) certification.
- A genuine commitment to compassionate, patient-centered hospice and palliative care.
- Willingness to work the day-shift schedule, including the every-twelfth-weekend and three-holiday-per-year rotations.
- Ability to travel to patient homes throughout the Waukesha area.
