This Full Time position provides essential in‑home support to patients receiving hospice care, helping them maintain comfort and dignity while assisting families during a challenging time.
What You'll Do
- Assist clients with personal hygiene, grooming, dressing, and bathing
- Support mobility and safe transfers using proper body mechanics
- Monitor and record vital signs, intake/output, and any changes in condition
- Provide medication reminders and help with basic wound care as directed
- Offer companionship, emotional support, and active listening to patients and families
- Maintain a clean and safe patient environment, including light housekeeping tasks
- Communicate observations and concerns to the hospice nursing team
- Follow each patient’s individualized care plan and adhere to all agency policies
- Complete accurate and timely documentation in the electronic health record
What We're Looking For
- Current Home Health Aide certification or Certified Nursing Assistant license in California
- Valid CPR/BLS certification
- Compassionate, patient‑centered attitude with strong interpersonal skills
- Reliable transportation and ability to travel throughout Palm Springs and surrounding areas
- Ability to lift and assist patients safely (up to 50 pounds with assistance)
- Basic computer skills for charting and reporting
- Successful background check and drug screening
- Flexibility to work varying shifts, including evenings and weekends as needed
