About the Role
As a Case Manager on PATH's Health and Psychosocial Support Team, you'll work directly with program participants to address their healthcare and housing needs through individualized case management. Your goal is to help clients move beyond homelessness by connecting them to essential services, coordinating their care across multiple providers, and supporting their progress toward self-sufficiency. You'll engage participants across all phases of their journey—whether on the streets, in interim housing, transitional housing, or permanent supportive housing—providing consistent advocacy and resource connection.
What You'll Do
- Conduct comprehensive intake assessments and collaborate with participants to create individualized service plans that address housing, income, medical needs, mental health, substance use, employment, financial resources, and social supports
- Connect participants with healthcare, social services, and community resources that align with their personal goals and service plan objectives
- Complete evidence-based assessments (such as the VI-SPDAT) to evaluate strengths, barriers, and priority needs
- Conduct structured 90-day reviews to track progress and adjust service strategies as needed
- Maintain thorough, timely documentation in participant files and program databases, adhering to all regulatory and organizational standards
What We're Looking For
- Preferred: Two years of experience in healthcare, case management, homelessness services, or direct work with vulnerable populations
- Strong ability to organize information, identify trends, and make informed decisions with attention to detail
- Excellent written and verbal communication skills that allow you to engage diverse audiences with clarity and professionalism
- Proactive problem-solver who takes initiative, thinks creatively, and adapts to complex situations
- Team player who builds collaborative relationships with colleagues, participants, and community partners
- Commitment to ethical practice, professional boundaries, honesty, and integrity
- Valid California driver's license, reliable transportation, proof of insurance and vehicle ownership, and ability to meet PATH insurance requirements (driving is essential to this role)
- Employment authorization for the United States; candidates must complete a tuberculosis test, background screening, and drug screening as a condition of employment
- Ability to obtain or current CPR/First Aid certification
