Responsibilities

• Conduct home health visits to assess patient condition, take vital signs, and track progress toward care goals
• Administer prescribed medications and treatments while educating patients and families on proper techniques
• Assist with personal care, wound care, and basic medical procedures as directed by the supervising registered nurse or physician
• Maintain clear, timely documentation of visits, observations, and interventions in the patient record
• Collaborate with the multidisciplinary team, report changes in status promptly, and help implement care plan updates
• Promote patient independence and safety within the home environment during each scheduled encounter

Who We're Looking For

• Current and unrestricted LPN license meeting state requirements for practice in the relevant areas among the 6 Locations
• Demonstrated ability or readiness to provide care in home health settings with sound clinical judgment and attention to protocol
• Strong communication skills for working with patients, families, and other care providers
• Reliable transportation and willingness to travel to patient homes as part of the full-time schedule
• Commitment to confidentiality, professional ethics, and delivering compassionate, competent support consistent with Home Health FT LPN standards
