Position Summary

Working under nursing direction, you become a trusted part of each patient’s daily routine, focusing on safety, dignity, and clear communication. Your Responsibilities
- Help adult patients with bathing, dressing, grooming, oral care, and toileting in the comfort of their own homes.
- Assist with safe transfers, positioning, walking, and range-of-motion exercises according to the care plan.
- Measure and document vital signs, weight, and intake accurately for review by the clinical team.
- Offer friendly companionship that reduces isolation and supports emotional well-being.
- Complete light housekeeping, meal setup, and laundry tasks that keep the living environment clean and orderly.
- Watch for changes in physical or mental status and report observations promptly to the supervising nurse.
- Follow each individualized care plan exactly and complete all required visit documentation before leaving the home.

Requirements
- Active Licensed Nurse Assistant (LNA) credential recognized in Vermont.
- Clear verbal and written communication skills with patients, families, and clinical partners.
- Ability to work autonomously in private homes while adhering to established protocols.
- Valid driver’s license, reliable vehicle, and willingness to cover assigned territories around Camp Johnson, Chittenden County.
- Genuine compassion, patience, and respect for adult patients of varying abilities.
- Current CPR or basic life support certification preferred.
