About the Role

As a Full Time Physical Therapy Assistant (PTA) with CommuniCare Rehab in Carmel, you'll deliver hands-on rehabilitation care under the direction of a licensed physical therapist. Working on-site at Bridgewater Healthcare Center, you become a trusted member of an in-house clinical team rather than an outside contractor — giving you the consistency and connection to make a genuine difference in each resident's recovery journey.

Key Responsibilities

• Carry out physical therapy treatment plans developed by the supervising physical therapist
• Guide residents through therapeutic exercises, mobility training, and functional activities
• Track patient progress and report observations and outcomes to the therapy team
• Maintain accurate, timely documentation in line with facility and regulatory standards
• Support a safe, encouraging, and respectful environment for every resident

Qualifications

• Completion of an accredited Physical Therapist Assistant program
• Current PTA licensure or eligibility to practice in the state of Indiana
• Commitment to compassionate, resident-centered rehabilitation care
• Strong communication skills and the ability to collaborate within an in-house clinical team
