About the Role
As a Physical Therapist (PT) with CommuniCare, you will join an in-house clinical team at Martinsburg Healthcare Center, located at 209 Clover St, Martinsburg, WV 25404. Rather than working through an outside agency, you become a true team member, giving you the consistency and connection that allow you to make a direct, lasting difference in the recovery of every resident you treat. This is a full-time, on-site position.
Key Responsibilities
- Evaluate residents and design individualized physical therapy treatment plans tailored to their rehabilitation goals.
- Deliver skilled therapy interventions that improve mobility, strength, function, and overall quality of life.
- Track progress closely and adjust care plans as residents advance toward their goals.
- Partner with the broader in-house care team to coordinate well-rounded, resident-centered treatment.
- Maintain accurate, timely clinical documentation in line with professional and regulatory standards.
Qualifications
- Degree in Physical Therapy from an accredited program.
- Active and valid Physical Therapist (PT) licensure in the state of West Virginia, or eligibility to obtain it.
- A genuine commitment to compassionate, resident-focused rehabilitative care.
- Strong communication and collaboration skills for working within a multidisciplinary team.
