Build meaningful patient relationships as a Nurse Practitioner or Physician Assistant at WelbeHealth. This full-time Primary Care Provider position in North Hollywood, CA puts you at the center of coordinated senior care delivered through our PACE program.

What This Role Involves

You'll become a key clinical leader in a holistic, team-based approach to geriatric medicine. Rather than managing high patient volumes, you'll cultivate sustained partnerships with seniors and their families while leading integrated care coordination. Working within a multidisciplinary team of physicians, nurses, social workers, therapists, and specialists, you'll focus on comprehensive assessment, personalized care planning, and supporting seniors' independence and quality of life.

Responsibilities

  • Conduct comprehensive clinical assessments—initial evaluations, periodic check-ups, and urgent visits—to guide continuous participant care and monitoring
  • Develop individualized care plans in close partnership with physicians and the interdisciplinary care team
  • Coordinate care transitions, manage placements in skilled nursing facilities, and arrange discharges back to home-based settings
  • Make regular in-person visits to participants in their homes and residential settings based on clinical need
  • Analyze health outcomes and utilization data to support evidence-based practice improvement and quality initiatives

What You Bring

  • Active RN license and current Nurse Practitioner or Physician Assistant certification with prescriptive authority
  • Minimum two years of direct clinical experience with frail, elderly, or geriatric populations
  • Demonstrated knowledge of chronic disease management and geriatric best practices
  • Valid driver's license and reliable personal transportation
  • Willingness to share rotating on-call responsibilities, including nights and weekends
  • Completion of an accredited nursing degree program
