About the Role

Theoria Medical advances post-acute care by embedding physician-led expertise directly into skilled nursing facilities nationwide. Our mission is bringing comprehensive, patient-centered clinical services to residents rather than requiring them to leave their care settings. In this temporary Provider Liaison Medical Assistant role, you'll coordinate specialty provider consultations, facilitate remote clinical visits, and navigate the complex transition-of-care process while reinforcing clinical education that drives measurable health improvements.

What You'll Do

• Schedule and facilitate both in-person and telemedicine provider visits, preparing residents with necessary documentation and positioning support
• Maintain and update electronic health records with patient histories and clinical information that supports care planning
• Shepherd patients through care transitions by coordinating referrals, arranging follow-up appointments, and facilitating prior authorization requests
• Educate residents on clinical topics including nutrition, fall safety, medication adherence, and wellness practices
• Distribute provider-approved educational materials and route questions and clinical concerns to licensed nursing staff or physicians
• Complete accurate documentation in electronic systems that ensures regulatory compliance and supports continuous quality improvement
• Attend interdisciplinary care team meetings and provide additional patient context to support coordinated decision-making
• Develop working relationships with facility staff, hospital departments, case management teams, and administrative leaders to streamline care coordination

Qualifications

• Graduate of an accredited Medical Assistant program; Certified Medical Assistant (CMA) credential preferred
• Prior clinical experience as a Medical Assistant, or background in care coordination, health plan operations, or hospice liaison roles
• Demonstrated proficiency charting in electronic medical record systems
• Strong interpersonal skills with ability to connect professionally with patients, families, and healthcare teams
• Knowledge of geriatric medical practice and age-related healthcare considerations
• Advanced computer competency including email, spreadsheets, and data entry applications
• Clear verbal and written communication skills
• Detail orientation, autonomous problem-solving, and commitment to quality outcomes
• Ability to travel locally (up to 30 miles), lift up to 20 pounds independently, and stand during extended facility rounds
• Professional demeanor and ethical commitment to patient-centered care

Working at Theoria Medical

You'll join a physician-led team transforming how skilled nursing facilities deliver specialized care. This temporary, full-time position offers 6-hour facility shifts (typically starting between 7–10 a.m.) with no on-call or overnight requirements. The compensation range is $19–$24 per hour. Theoria Medical requires background screening and pre-employment drug testing for all hired candidates.
