Clinical Pharmacist – Cardiology / Value-Based Care

Responsibilities

We are representing a confidential healthcare organization in their search for a full-time, contract Clinical Pharmacist specializing in cardiology and value-based care. This remote role focuses on optimizing guideline-directed medical therapy and supporting medication titration for high-risk cardiac patients. You will not be dispensing medications—instead, you'll function as a clinical partner embedded within an interdisciplinary care team.

Your day-to-day responsibilities will include:

• Reviewing patient medication profiles for cardiology populations including heart failure, coronary artery disease, atrial fibrillation, hypertension, and chronic kidney disease
• Evaluating appropriateness of guideline-directed medical therapy based on ACC/AHA standards and payer quality programs
• Identifying gaps in therapy, contraindications, drug interactions, and adherence barriers
• Making evidence-based recommendations for medication initiation, optimization, and titration in collaboration with providers and nursing staff
• Conducting telephonic medication reviews and patient education sessions
• Educating patients on medication purpose, benefits, side effects, and helping them recognize red-flag symptoms
• Working closely with RN Care Managers and Nurse Navigators to align medication plans with care goals
• Communicating clinical recommendations clearly to cardiologists and advanced practice providers
• Participating in interdisciplinary case reviews and care planning
• Documenting recommendations and patient interactions promptly in care management platforms
• Contributing to performance on medication-related quality measures and helping reduce avoidable adverse events

What We're Looking For

Required qualifications:

• PharmD degree
• Active Texas pharmacist license in good standing
• Multi-state pharmacist licensure required (remote role, not restricted to Texas residency)
• Minimum 3–5 years of clinical experience with a strong cardiology focus
• Demonstrated working knowledge of guideline-directed medical therapy and cardiac medication titration
• Comfort working within a nurse-led care management model
• Strong telephonic patient communication skills
• Ability to interact professionally and confidently with cardiology providers
• Experience engaging directly with patients and collaborating with physicians and advanced practice providers
• Time zone compatibility—must be able to work during Central Time business hours
• Exceptional written and verbal English communication skills

Preferred qualifications:

• Prior experience in heart failure or cardiology clinics
• Experience working within clinically integrated networks or value-based care models
• Familiarity with ACC and AHA guidelines and payer quality programs
• Experience documenting in electronic medical records or care management platforms
• Prior remote or telehealth patient engagement experience

• Personal Windows/Mac laptop with 16 GB RAM minimum
• Windows 11+ or supported macOS with Google Chrome
• External monitor, mouse, and keyboard
• Working webcam, microphone, and headset/audio
• Reliable broadband: 100 Mbps download / 10 Mbps upload minimum
• Dedicated, private workspace suitable for handling PHI with secure internet connection and ability to conduct confidential patient communications in compliance with HIPAA

Pay

Pay via Get A Job.ai: $60–$65 USD per hour for this full-time, contract position (40 hours/week, Monday–Friday).
