Uptiv Health : Patient and Provider Guide (PPG)

About This Opportunity

Get A Job.ai is representing a confidential healthcare organization that is transforming infusion care delivery for patients with complex chronic conditions. We are seeking a Patient and Provider Guide (PPG) to join their remote support team.

This is a fully remote, full-time position based in the United States. You will serve as a critical liaison between patients, referring providers, and the care team, ensuring seamless communication and scheduling coordination.

Responsibilities

• Serve as the first point of contact via phone, email, and text platform, responding to inquiries from referring providers and patients in a timely, accurate, and compassionate manner
• Manage patient scheduling and onboarding processes, including initial appointments and rescheduling requests
• Handle all inbound and outbound patient and provider communications
• Meticulously document all patient and provider interactions and take appropriate follow-up actions
• Navigate and maintain accurate records in Electronic Health Record (EHR) systems
• Analyze, prioritize, and synchronize multiple high-impact tasks in a fast-paced environment

What We're Looking For

Required Experience:

• 2+ years of clinical patient phone support experience in payer, hospital system, digital health, clinic-based, or revenue cycle management environments
• Detailed familiarity with injection and infusion therapy
• Strong understanding of clinical processes and protocols
• Proven ability to work effectively in a fully remote environment using various technology platforms

• Proficiency with Electronic Health Record (EHR) systems
• Excellent multitasking and priority management skills
