About the Role

As a Care Coordinator, RN Field Based with Humana, you'll serve on the Humana Healthy Horizons team in Indiana, working directly with members in their homes and communities. Your role centers on understanding each member's unique health needs, developing personalized care plans, and connecting them with appropriate services and support systems. You'll be a trusted healthcare partner, guiding members and their families through complex healthcare decisions and ensuring they have access to the resources needed to achieve and maintain their best possible health.

Key Responsibilities

• Assess and evaluate individual member health needs, challenges, and goals
• Develop and coordinate comprehensive care plans tailored to each member's circumstances
• Connect members with community resources, social services, and clinical support
• Conduct field visits to members' homes and meet with individuals in community settings
• Communicate and collaborate with healthcare providers, social workers, and family members
• Monitor member progress and adjust care coordination strategies as needed
• Document interactions and maintain accurate member records

Requirements

• Active Registered Nurse (RN) license in Indiana
• Ability to travel and conduct field visits within the service area
• Strong communication and interpersonal skills
• Experience in care coordination, case management, or community health nursing
• Proficiency with electronic health records and healthcare documentation systems
• Compassion for working with diverse populations and complex health conditions
