Care Coordinator, RN Field Based

Humana is seeking a Care Coordinator, RN Field Based to join the Healthy Horizons team in Woodbridge, Monroe County, Indiana.

About the Role

This full‑time position focuses on supporting members in their homes and communities, helping them achieve and maintain optimal health through personalized assessment scores by assessing and resources that promote wellbeing. You will work closely with patients, families, and interdisciplinary teams to develop and monitor care plans that align with each member’s goals.

What You'll Do

• Conduct comprehensive assessments of members’ health, social, and functional needs during scheduled field visits.
• Develop, implement, and adjust individualized care plans in collaboration with members, caregivers, and clinical providers.
• Facilitate access to community resources, medical services, and support programs that enhance member wellness.
• Provide education and coaching to members and families on disease management, medication adherence, and preventive health practices.
• Monitor member progress, document encounters accurately in electronic health records, and report outcomes to the care management team.
• Coordinate transitions of care, including hospital discharges and specialist referrals, to ensure continuity and reduce readmissions.
• Participate in quality improvement initiatives and adhere to Humana’s compliance, privacy, and safety standards.

What We're Looking For

• Active Registered Nurse (RN) license in the state of Indiana.
• Minimum of two years of clinical nursing experience, preferably in case management, home health, or community-based care.
• Strong assessment and critical‑thinking skills with the ability to prioritize competing needs.
• Excellent interpersonal and communication abilities to build trust with diverse populations.
• Proficiency with electronic medical record systems and basic data‑entry tasks.
• Ability to travel independently within the assigned geographic area and maintain a valid driver’s license.
• Commitment to delivering compassionate, culturally sensitive care that promotes member empowerment.
